It can take some time to complete an intake assessment form with the client. While it should be completed as soon as possible after the client is identified, it may be acceptable to complete an individual’s intake over the course of several client meetings. One to four meetings over the course of one to two weeks will be sufficient to complete an Intake Assessment Form.
If it does take several meetings to complete the form, be sure that any information collected in early sessions is not changed in later sessions, even if the client offers updated information. For example, if the client reveals in the first session that he is a smoker but tells you he has quit in the second session, the information on your client quitting smoking should not appear on the Intake Form. It should, however, be recorded in progress notes, chart notes, or the electronic medical record (EMR). This is particularly important in situations where the new information affects service delivery.
