STEPS (Science-based Translation of Effective Program Strategies) to Care was developed by the Education Development Center, Inc., in partnership with the New York City Department of Health and Mental Hygiene, with funding from the Centers for Disease Control and Prevention, U.S. Department of Health and Human Services. This online toolkit aims to support and improve agency practices related to HIV linkage to care, retention, and re-engagement in care through the use of three strategies: Patient Navigation, Care Team Coordination, and HIV Self-Management.
